How to Register as a Patient
Complete a Patient Registration Form
The registration process will take around 15 minutes to complete. You will need to complete the process in one session.
Other ways to register
If you do not wish to Register Online you can download and complete the following form:
- A Register with a GP Surgery (PRF1) form completed with your NHS Number (we are unable to register you without your NHS Number, you can find your NHS Number using the NHS website, obtain it from your previous GP Surgery or from your repeat prescription).
When the form is completed bring it into reception to complete the registration process.

Register your New Baby
Please register your new baby at the practice as soon as possible. Download and fill out the Register with a GP Surgery (PRF1) form, or pick up a copy from reception. Return the completed form to reception and include your baby’s NHS Number, which can be found in their Red Book.
Once registered, we’ll send you an appointment for a mother and baby post natal check. This will take 50 minutes; 30 minutes for your baby’s check and 20 minutes for yours. Your baby’s immunisations will start at 8 weeks and will be done at this appointment.
Don’t forget to bring the Red Book. If you can’t make the appointment, let reception know as soon as possible. Please note, rescheduling may mean waiting up to 4 weeks for the next available slot.
Register as a Temporary Resident
Our doctors are happy to provide medical advice and care to patients who are temporarily residing in the Practice area. If you require treatment, please inform reception that you are a temporary resident.
To register, please bring your passport and a completed Temporary Resident Registration (GMS3) form to our surgery reception. Paper copies of the GMS3 form are also available from reception.
How to Find Your NHS Number
An NHS number is a 10 digit number, like 485 777 3456.
Your NHS number is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records.

Accountable Named GP for All Patients
All patients registered at Russell House Surgery have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.
If you are unsure of who is your named Doctor please ask at reception.
New patients are allocated a Doctor on Registration.
